We will then be in touch with an invoice and further instructions. We are so excited about this program, and we know your dog will be excited, too! Please make sure you are my friend on Facebook (Shannon Heggem, but you already knew that…!) so that we can add you to the Facebook Group for Playcare! This is where we communicate with all of our attendees. And we upload pictures and videos while each session is going on-it is so fun!

Drop-offs and Pick-Ups

Rules for drop offs and pick ups are the same as they are for boarding: only you or your spouse/partner is allowed in the kennel AT ANY TIME. Adult children, children children, neighbors, friends, relatives, and other Luxury Unleashed customers ARE ALSO NOT ALLOWED to drop off or pick up your pet, nor are they allowed to accompany you while doing so. By completing this field, you agree and consent to being filmed by our security cameras at all times while in the kennel and/or drop off areas. Dogs are to be dropped off any time AFTER 6 am, up until 9 am. PLEASE do not show up after 9 without text permission from Shannon as it will disrupt the group! Dogs may be picked up any time after 2:30pm. Playcare ends at 2pm. HOWEVER, we have treat time and rest time, and we also need time to get all of the dogs ready to be picked up. Dogs will be available to be picked up until 6:30. *If you need to leave the dog later, no problem! Please text Shannon and let her know in advance! IN ORDER TO AVOID BEING CHARGED A SIGNIFICANT SANITATION FEE, PLEASE notifiy Shannon BEFORE bringing your dog to PLAYCARE if ANY of the following have occured: (Yes, now that I have your attention…WE NEED TO KNOW IF YOUR DOG HAS BEEN SICK BEFORE YOU BRING HIM HERE! Been exposed to any other dog who was sick

Been scratching more than normal

Cancellations and Credit Policies

PRICING & PAYMENT: Space for PLAYCARE is very limited. Passes must be purchased in order to secure a spot. Monthly passes are for four CONSECUTIVE Playcare sessions, and must be used as such. (If, for some reason your application is not approved, you will receive full refund.) ATTENDANCE POLICY: Attendees who are unable to attend for any reason will have 50% of the session fee subtracted from their pass for each missed session. (Please note that this includes planned absences.)ENROLLMENT POLICY: You may terminate your dog’s enrollment in Playcare anytime, but it is best to do after your passes have been utilized as no refunds will be issued. Re-enrollment into the program is not guaranteed, and is subject to availability. Legal

GENERAL TERMS: Luxury Unleashed LLC will exercise reasonable care for the safety of your pet and to keep the boarding and day care premises safe and secure. Luxury Unleashed LLC's liability for any boarded pet will not exceed $25. Pet owner is solely responsible for any property damage or bodily injury inflicted by the boarded animal. Luxury Unleashed LLC cannot guarantee against accidents. We will not be liable for loss or damages to pet’s belongings.(Required)

PLAYCARE REQUIREMENTS: Pets may be dropped off with nylon or leather collars or no collar at all (choke collars, flea collars must be removed.) Pets are usually not fed during PLAYCARE sessions unless owners specify such. If the dog is to be fed, owner needs to provide usual food and treats. We will provide a transport container for the food. If pet is trained to use training pads, these must be provided by owners. PLEASE do not bring your pet’s dishes, and please do not put any food in the enclosure or feed the pet when you drop him off.(Required)

MEDICATION & TREATMENT AUTHORIZATION: Prescription medications must be provided in the original container with the veterinarian’s Instructions, It will be administered as prescribed. Luxury Unleashed LLC is authorized by the owner to seek veterinary advice and care, including but not limited to emergency care, at their discretion and at the owner’s sole expense. Luxury Unleashed LLC will attempt to contact the owner before obtaining this care if possible, but this document serves as full authorization to obtain veterinary care for the pet at the owner’s expense, whether or not owner has been contacted.(Required)

This contract shall be legal and binding for one year from the following date, and will include any and all boarding and PLAYCARE during that period. This contract contains entire agreement, and shall be binding upon heirs, administrators, personal reps or assigns of either party. Any controversy arising from this agreement will be settled by arbitration according the Montana Arbitration Laws, with the expense of such paid by the pet owner, regardless of prevailing party.
