This application is detailed and intentional. Every question exists for a reason. Please allow 15 to 20 minutes to complete it. One application per dog. If you see an error in red, correct it and resubmit. After submitting successfully you will be taken to a confirmation page and will receive a confirmation email. If you do not receive the confirmation email, we did not receive your application.

A Note About This Application: The questions in this form exist in part to make sure we are the right fit for your dog, and that your dog is the right fit for us. If we are not the right fit, we will not accept your application, and instead refer you to another facility which is.

*A Note About Same-Day and Emergency Boarding Requests: We simply cannot guarantee same-day availability for new clients. Dogs must be current on Distemper/Parvo, Rabies, and Bordetella. If you have had them updated recently, please note that there MUST BE AT LEAST ten days inbetween the vaccinations being given and your boarding date. This is an insurance requirement.(Required)

Reservation Changes

We completely understand that travel plans change. Flight delays, extended trips, early returns. We have seen it all and we are genuinely flexible.

Instead of texting us for permission, simply go to the Existing Clients tab on our website and use the Change or Cancel Reservation link. That way nothing gets lost, your change is documented, and we can plan accordingly for your dogs already in our care. We are very flexible and will always work with you!

Specialized Care We specialize in special needs dogs: elderly dogs are always welcome, dogs with mobility challenges, chronic health conditions, and complex care needs are not a burden to us. Has your dog received deworming treatment from a veterinarian in the past six months? Regular deworming is essential for dogs, and some owners may not be aware of this. A simple call to your veterinarian can provide you with a one-time pill or powder to be mixed into your dog's food. It's a straightforward process! We emphasize this because we value your pet's ongoing health and genuinely care about our guests.(Required)

Food and Feeding

FEEDING INSTRUCTIONS: To ensure that we can lovingly care for each pet with precision please also provide WRITTEN FEEDING INSTRUCTIONS ALSO with their food, unless you plan to pre-portion meals into Ziplock bags (which we love!), simply label each bag with your dog’s name and frequencyn(especially important if you have multiple pets). We’ll happily return the bags for reuse! Thanks so much for your cooperation, we appreciate your help in keeping our furry guests happy, healthy, and wonderfully spoiled!

Drop Off Procedures

What to Bring

  • Dry dog food in containers under 10 pounds each
  • Any refrigerated food in Ziploc bags only
  • Medications in original labeled containers from your veterinarian
  • Pill pockets or medication delivery treats if your dog takes oral medication
  • You may bring one clean bed, must be 3 feet by 4 feet or smaller
  • You may bring one clean blanket
  • You may bring soft toys
  • You may bring Kong toys and fillers

What to Leave at Home

  • No food and water dishes of any kind. We provide our own sanitized bowls.
  • No rawhide, antlers, hard plastic chew toys, or real bones
  • No beds larger than 3 feet by 4 feet. They will not be used and a $50 fee applies.
  • No Rubbermaid or hard sided containers for the refrigerator
  • No large bags of dog food or anything over 10 pounds
PAYMENT: You will receive an online invoice for your boarding fees prior to your departure date. This invoice must be paidbye the due date in order to hold your boarding reservation and to avoid a late payment fee. We will issue a door code for access (you will learn more about this after your application has been approved. We will provide detailed instructions regarding access to our drop off and pick up area).(Required)

A Note About Photos During Your Dog’s Stay

We want to be upfront about something before your dog stays with us. Our insurance carrier has implemented a policy that prohibits us from sending photos of individual dogs during a boarding stay. The concern is that a photo of your dog could inadvertently reveal another dog in the background, and by extension reveal that another client is away from home.


We did not make this rule. We do not like it. We genuinely hate it, a whole bunch. But we do not have a choice and we would rather be honest with you about why than simply say no.

Here is exactly what happens next so you are never left wondering.

If your application is NOT a good fit, you will hear from us right away. We will let you know promptly and where possible point you toward someone who may be a better match for your dog.


If your application looks good:

Nothing moves forward until we receive your vaccination records directly from your veterinarian. Not from you. From the vet.

We are a two-person operation. Please understand that we may not respond immediately upon receiving your vet records, and we may not respond the same day you submitted your application. We will always reach out before your boarding dates. You will not be left without an answer.

We do not overbook. Ever. After more than three decades in this industry we have made a deliberate choice to take on only as many dogs as we can genuinely care for at the level our clients expect.


About photos during your dog’s stay:

Our insurance carrier prohibits us from sending photos during your dog’s stay. We are genuinely frustrated by this rule. It is not ours. We do not have a choice, and we would rather be honest with you about why than simply say no. We appreciate your understanding more than you know.


If your dog becomes ill during their stay:

If we have time to reach you we will, but in a true emergency your dog comes first. We are authorized to seek veterinary care on your behalf at your sole expense.


Your onboarding process:

  1. You will receive an email with a virtual tour of our facility.
  2. We will send a video walkthrough of our drop-off process.
  3. We will schedule a welcome call to answer your questions and make sure we have everything we need to give your dog an exceptional stay.
  4. You will receive your invoice. Once paid, your door code is issued and you are confirmed.
  5. On drop-off day, after settling your dog in, please send us a quick text to let us know you are on your way out.

We are genuinely glad you are here. We do not take that lightly.

Legal

Luxury Unleashed LLC will exercise reasonable care for the safety of your pet and to keep the boarding premises safe and secure. Luxury Unleashed LLC's liability for any boarded pet will not exceed $25. Pet owner is solely responsible for any property damage or bodily injury inflicted by the boarded animal. Luxury Unleashed LLC cannot guarantee against accidents. We will not be liable for loss or damages to pet's belongings.(Required)
VACCINATIONS AND HEALTH: All animals must be current on vaccines for Rabies, DHLPP and Bordetella (kennel cough). Vaccination records must be provided to Luxury Unleashed LLC directly from the veterinarian prior to boarding. Records may be emailed to shannonheggem@gmail.com. All animals accepted for boarding must be free of fleas and ticks. Owner will be charged a $300 fee for any animal that is dropped off with parasites, which must be paid before the animal will be released. Owner hereby represents that this pet has not been exposed to Kennel Cough, Rabies, Bordetella, Distemper, or Parvo within the last 30 days.(Required)
BOARDING REQUIREMENTS: Owners are required to provide their dog's usual food in approved containers. If your pet uses training pads, these must be provided by the owner. Owners may bring treats, clean blankets, clean beds within the approved size limit, and toys as outlined in this application. We will make every effort to ensure that whatever you bring with your pet will be returned.(Required)
MEDICATION AND TREATMENT AUTHORIZATION: Prescription medications must be provided in the original container with the veterinarian's instructions. Medication will be administered as prescribed. Owner must provide pill pockets, pill-hiding treats, or other medication delivery method if their dog requires oral medication. If these are not provided, a $5 per day charge will apply for Luxury Unleashed LLC to provide them. Luxury Unleashed LLC is authorized by the owner to seek veterinary advice and care, including but not limited to emergency care, at their discretion and at the owner's sole expense. Luxury Unleashed LLC will attempt to contact the owner before obtaining this care if possible, but this document serves as full authorization to obtain veterinary care for the pet at the owner's expense, whether or not the owner has been contacted.(Required)
PAYMENT TERMS: Owner agrees to pay current boarding rates as listed on the website, and all specialty service charges in advance. No cash refunds will be issued for any reason. Boarding credit valid for one year from the date of issue will be issued for cancellations with 24-hour notice. No credit will be issued for early returns or late departures. You will receive an online invoice for your boarding fees prior to your departure date. This invoice must be paid within three days of receipt in order to hold your reservation and to avoid a late payment fee. Access to your pet will be denied and your pet will not be released until all charges are paid in full.(Required)
DAILY RATE ACKNOWLEDGMENT: I understand that I will be charged a daily rate for boarding, not a nightly rate like a hotel. I understand that all fees must be paid online in advance.(Required)
ACCESS RESTRICTIONS: I understand that no one besides myself or my spouse/partner is allowed in the drop-off/pick-up lounge to access my pet at any time. I hereby agree that I will not give access to anyone else, including my children (not even adult children), relatives, employees, friends, neighbors, or other Luxury Unleashed LLC clients. Unauthorized entry by any third party constitutes trespassing and may result in criminal charges.(Required)
